VO2 Max: The Single Number That Predicts How Long You'll Live

If you could only track one number for predicting your long-term health and longevity, it would not be your cholesterol. It would not be your blood pressure. It would be your VO2 max.

A major 2018 study published in JAMA Network Open followed over 120,000 patients for a decade and found that low cardiorespiratory fitness — measured by VO2 max — was associated with a higher risk of death than smoking, high blood pressure, diabetes, and high cholesterol. Not comparable to. Higher than.

That is a remarkable finding, and most people have never heard of VO2 max.

What VO2 Max Actually Means

VO2 max is the maximum rate at which your body can consume oxygen during intense exercise — measured in milliliters of oxygen per kilogram of body weight per minute (ml/kg/min). It is the best single proxy for your cardiovascular and respiratory system’s efficiency under stress.

A high VO2 max means your lungs are efficiently exchanging oxygen, your heart is pumping that oxygen-rich blood powerfully, and your muscles are extracting and using it well. Every one of those systems degrades with age — but the rate of degradation is not fixed. It is heavily influenced by how you train.

The average sedentary adult loses roughly 10% of their VO2 max per decade after age 25. Trained adults lose closer to 5% per decade. That gap compounds over time in ways that dramatically separate healthy aging from decline.

Why It Predicts Longevity So Well

VO2 max is essentially a stress test for your entire cardiovascular system. A body with high aerobic capacity is one where the heart, lungs, blood vessels, and mitochondria are all working well together. When any of those systems starts to break down — early heart disease, reduced lung elasticity, vascular stiffening — VO2 max is often one of the first markers to fall.

Research from the Cooper Center Longitudinal Study tracked thousands of patients over decades and found that going from “low” to “moderate” fitness category on a VO2 max test reduced all-cause mortality risk by 46%. Going from “moderate” to “high” reduced it by another 29%. No medication has demonstrated that kind of risk reduction across that population size.

Increasing your VO2 max also reduces risk of:

  • Dementia and cognitive decline (aerobic fitness is one of the strongest protective factors for brain health in aging)
  • Type 2 diabetes
  • Cardiovascular disease
  • Depression and anxiety

How to Estimate Your VO2 Max Without a Lab

A clinical VO2 max test requires a treadmill, a mask, and a sports medicine lab. But there are reliable estimates you can do yourself.

The Rockport Walking Test (Most Accessible) Walk 1 mile as fast as possible on flat ground. Record your time and your heart rate immediately at the finish. Plug into this formula:

VO2 max ≈ 132.853 − (0.1692 × body weight in kg) − (0.3877 × age) + (6.315 × sex, where male = 1, female = 0) − (3.2649 × mile time in minutes) − (0.1565 × heart rate at finish)

This gives you a rough estimate within about 5–10% of a lab test.

The Simpler Gauge: Stair Climb Climb four flights of stairs (about 60 steps) at a brisk pace. If you arrive at the top without breathlessness and your heart rate recovers within 1–2 minutes, you have reasonable aerobic fitness for your age. If you are significantly winded or it takes 5+ minutes to recover, aerobic capacity is worth prioritizing immediately.

Wearable Estimates Apple Watch, Garmin, Whoop, and most modern fitness wearables now estimate VO2 max from heart rate data. These are not perfectly accurate but they are directionally useful for tracking your trend over months of training. The trend matters more than the precise number.

If you are below “average” for your age, the research is clear: improving this number is likely the single highest-return investment you can make in your long-term health.

How to Improve It

VO2 max responds to two types of training stimulus, and both are necessary for optimal improvement.

Zone 2 Training (The Foundation)

Zone 2 is the aerobic intensity where you can maintain a conversation but it requires some effort — roughly 60–70% of your maximum heart rate. It is the intensity of a brisk walk, a relaxed jog, an easy bike ride. Done consistently for 45–60 minutes, 3–4 times per week, Zone 2 training:

  • Increases mitochondrial density in slow-twitch muscle fibers
  • Improves fat oxidation (your ability to burn fat as fuel)
  • Lowers resting heart rate over weeks
  • Builds the aerobic base that makes interval training more effective

Most people who “don’t like cardio” dislike it because they were never told about Zone 2. They defaulted to moderate intensity — not hard enough to build fitness, hard enough to be unpleasant. Zone 2 is genuinely sustainable. A 45-minute walk at a pace where you’re slightly warm and breathing deeper than resting is Zone 2.

High-Intensity Interval Training (The Stimulus)

Once you have a Zone 2 base, adding HIIT 1–2 times per week produces the acute stimulus that forces VO2 max upward. The intervals need to be hard enough to push you to near-maximum heart rate — 85–95% — for short periods of 30 to 90 seconds. Your cardiovascular system adapts to those peaks by increasing stroke volume and improving oxygen extraction.

The Norwegian 4×4 Protocol

This is the most evidence-backed VO2 max protocol in the sports science literature. A study from the Norwegian University of Science and Technology found it superior to other training methods for improving VO2 max in both healthy adults and cardiac patients.

  • Warm up for 10 minutes at moderate intensity
  • 4 intervals of 4 minutes at 85–95% max heart rate
  • 3 minutes of active recovery between intervals (easy jogging or walking)
  • Cool down for 5 minutes

Done twice per week, alongside 2–3 Zone 2 sessions, most adults see measurable VO2 max improvement within 6–8 weeks.

The Honest Reality

You do not need to become an endurance athlete. Going from the “low” to “average” fitness category for your age cohort — a modest but real improvement — produces most of the mortality reduction benefit the research documents. That is achievable for almost everyone with consistent Zone 2 walking and two HIIT sessions per week.

The discomfort of cardio is real. So is the discomfort of a decade of declining function. The research on VO2 max makes the tradeoff unusually clear.
